About St. Jane de Chantal
St. Jane de Chantal Long Term Care Services was named after the Catholic Saint Jane de Chantal, who believed every person, regardless of health or age, was special. Today, inspired by her beliefs, we provide a loving, person-centered environment for our residents to call home. We embrace a holistic approach to care that emphasizes the mind, body and spirit connection.
Services
Long Term Care
Since 1985, St. Jane de Chantal has been providing specialized care. St. Jane is more than just a long term care facility to our residents—it’s home. The unit is comprised of a 66-bed dually certified Medicare/Medicaid nursing. St. Jane is the only nursing home in Nebraska accredited by the Commission on the Accreditation of Rehabilitation Facilities (CARF). St. Jane is accredited as a Person-Centered Long-Term Care Community under the Aging Services Program. Person-centered care is a philosophical approach to nursing home care that honors and respects the voice of our residents and those working closest with them.
- The expansive campus of the adjoining Madonna Rehabilitation Hospitals provides the expertise of rehabilitation professionals, an on-site lab, radiology and pharmacy, chapel with daily Mass and weekly ecumenical services, salon with beauty, barber and massage services, coffee shop and gift shop.
- Transportation services are provided for group outings and can be arranged for medical appointments. Professional nursing care is provided 24 hours a day, 365 days a year and gathering rooms for reunions and/or parties are available to residents and their families at no charge.
Transitional Care Unit
Licensed as a skilled nursing facility and accredited by CARF, the Rehabilitation Accreditation Commission, the Transitional Care Unit (TCU) is a 26-bed unit with all private rooms. TCU patients have access to Madonna’s exceptional medical rehabilitation services and nursing care, as well as a dedicated therapy gym and expanded activities area. Patients on the TCU receive therapy to meet individual needs five days per week, as well as skilled nursing care.
To be eligible for admission to the TCU, the patient must have a daily skilled nursing or skilled rehabilitation need which requires medical services, such as:
- Need for physical therapy, occupational therapy, and/or speech therapy less than 3 hours per day.
- Need for short-term rehabilitation for cardiac condition or following cardiovascular surgery.
- Nutritional support of a feeding tube with potential for discontinuation.
- IV medication administration
- Wound management
- Nursing observation/management and respiratory therapy for aerosol treatments.
- Face-to-face physician visit, if needed, up to 3 times weekly.
St. Angela Ventilator & Special Needs
The 30-bed unit uses a transdisciplinary team approach to care. Professional nursing and respiratory care is offered 24 hours a day, 365 days a year. The ventilator unit is one of only three in Nebraska and the special needs area is the only one of its kind in the state.
- The high nurse to patient ratio is ideal for complex patient care.
- The spacious rooms feature abundant natural light and scenic views of the courtyards.
- Assistive technology is available to help residents with limited mobility or muscle function to increase their computer skills.
- An on-site salon offers patients the convenience of beauty and barber services.
- Patients can choose from a wide array of extra-curricular activities, including Wii games, individual and group outings to concerts, the zoo, sporting events and more.
- A beautiful chapel features daily Mass and weekly ecumenical services.
- Alternative holistic therapy offerings include Tai Chi, yoga, aromatherapy, music therapy, in-room massage therapy, infrared mat therapy, light therapy, weighted blankets and pet therapy.
